Non-Managed Account

If you require a service that provides you with Payroll Only, you will need to complete the following forms:

• Registration Form – Non Managed  (Registration Form for Employer to Complete)
• PAS Starter Form – Non-Managed) (New Employee Starter Form)
• DP  dhPayroll – Engagement Letter  (Letter of Engagement for our Payroll Services)
• NEST Employer Terms and Conditions (Terms and Conditions for NEST pensions)
• NEST Direct Debit (Direct Debit to Collect Contributions due to NEST Pensions)
• DH Direct Payments Direct Debit  (Direct Debit for David Howard Invoices for Our Services) Optional – but a manual payment must be made quarterly if no Direct Debit is set up.
• Authorise a tax agent (64-8) – GOV.UK (www.gov.uk) (You will need to print and complete this form and send the hard copy to us at: David Howard Payroll, 1 Park Road , Hampton Wick, Kingston Upon-Thames, KT1 4AS)

Managed Account

If you require a service that offers Payroll and Payments, you will need to submit the following forms:

• Registration Form – Managed (Registration Form for Employer to Complete)
• PAS Starter Form – Managed (New Employee Starter Form)
• DPMA – dhPayroll – Engagement Letter (Letter of Engagement for our Managed Account/Payroll Services)
• NEST Employer Terms and Conditions (Terms and Conditions for NEST pensions)
• Authorise a tax agent (64-8) – GOV.UK (www.gov.uk) (You will need to print and complete this form and send the hard copy to us at: David Howard Payroll, 1 Park Road , Hampton Wick, Kingston Upon-Thames, KT1 4AS)

Managed Invoice Service

If you require Invoice/Agency Payments Only, you will need to submit the following forms:

• Registration Form – Invoice Only  (Registration Form for Employer to Complete)
• Registration Form for Invoice Payments (Registration Form for Self Employed Employees / Care Agency to Complete)
• DPMA – dhPayroll – Engagement Letter (Letter of Engagement for Employer to Complete)
